How do I submit my Certificate of Insurance?

That depends on if you already have insurance.

Either way, once you get a COI, then you MUST submit it using the form below.

1. Contact your insurance provider and request a COI for Texas Frightmare Weekend LLC. Our business address is 1629 Baxter Springs Dr, Justin, TX 76247
2. After you receive your COI, upload it at the form at the bottom of this page.
3. You are done!

I DO NOT currently have insurance.

2. When you receive your COI, upload it at the form at the bottom of this page.
3. You are done!

Deadline to submit your COI is May 4, 2026 at midnight CST. All vendors MUST supply their COI by this date or they will not be allowed to load in at the convention.
